Essential Aesthetics & Wellness Founding Member Policy

The Founding Membership is available only to the first 100 enrolled members.

Membership is personal, non-transferable, and may not be shared, assigned, or resold.

Members must be at least 18 years of age or have the consent of a parent or legal guardian where permitted by law.

Enrollment Fee: $100

The membership fee is paid once at enrollment.

There are:

• No monthly membership fees.
• No automatic renewals.
• No recurring billing.

Membership benefits are valid for 12 months from the enrollment date.

Upon enrollment, members will receive a Welcome Gift with an approximate retail value of up to $180. The specific products included may vary based on availability and professional recommendation.

The Welcome Gift:

• Is provided only once.
• Has no cash value.
• May not be exchanged for cash or credit.
• May not be returned once opened or used.

During the 12-month membership period, members receive:

• 15% discount on eligible facial services.
• 10% discount on eligible wellness services.
• 10% discount on eligible retail products.
• Complimentary professional skin consultation and wellness analysis. (Value: $150)
• Priority appointment scheduling.
• Birthday month special offer.
• Invitations to educational classes, wellness events and special offers.
• Early access to new products, services and technology.
• Exclusive member promotions throughout the year.
• Referral rewards.

Membership discounts do not apply to:

• Already discounted packages.
• Promotional specials.
• Gift certificates.
• Retail products (unless specifically stated).
• Medical services requiring outside provider fees.
• Taxes or gratuities.

Benefits may not be combined with other promotions unless specifically stated.

Essential Aesthetics & Wellness reserves the right to designate additional exclusions as necessary.

Appointments are required for all services.

Please provide at least 24 hours’ notice for cancellations or rescheduling.

Late cancellations or missed appointments may be subject to cancellation fees and may result in forfeiture of promotional offers associated with the appointment.

Founding Membership benefits expire 12 months after the enrollment date.

Unused benefits expire at the end of the membership period unless otherwise stated in writing.

The membership fee itself is not applied as a credit toward future services.

Approximately 30 days before expiration, members may be offered the opportunity to:

• Renew under the current membership program, if available.
• Upgrade to a new membership plan.
• Continue receiving services at standard pricing.

Renewal is optional.

Discounts apply only while the membership is active.

Discounts are calculated before taxes.

Discounts have no cash value and cannot be exchanged for cash or credit.

Membership is voluntary. Members may cancel with 30 days written notice.

Membership fees are non-refundable once the Welcome Gift has been received.

If the Welcome Gift has not yet been provided and no membership benefits have been used, refund requests may be reviewed on a case-by-case basis at management’s discretion, unless otherwise required by law.

Unused treatments expire according to membership guidelines.

If Essential Aesthetics & Wellness permanently ceases operations and permanently closes:

• Membership benefits will terminate on the business closure date.
• No future membership fees will be collected.
• Prepaid unused services will either:
  • be refunded on a prorated basis, or
  • be honored before the closing date whenever reasonably possible.

The Welcome Gift is considered fulfilled upon delivery and is not refundable.

The owner reserves the right to modify this policy as required by law.

Membership does not guarantee treatment eligibility.

Some services may require medical evaluation, approval or clearance before treatment by the supervising medical provider when applicable.

Treatment recommendations are individualized.

Results vary among individuals.

Membership is not medical insurance.

Services are cosmetic and wellness treatments and are not intended to diagnose, treat, cure, or prevent disease.

Membership provides access to discounts and exclusive benefits only.

Membership does not guarantee appointment availability, treatment outcomes, or continued availability of any specific product or service.

Essential Aesthetics & Wellness reserves the right to discontinue or replace products or services when necessary.

Essential Aesthetics & Wellness reserves the right to modify administrative procedures or membership benefits as needed for business operations, legal compliance, or service improvements.

Any material changes will not reduce benefits already purchased during the active 12-month membership period without appropriate notice, except where required by law.

Members agree to:

• Provide accurate health information.
• Update contact information when necessary.
• Follow pre- and post-treatment instructions.
• Treat staff and other clients respectfully.
• Comply with office policies.

Failure to comply may result in suspension or termination of membership benefits.

Members understand that aesthetic and wellness treatments involve potential risks and that individual results vary.

Members agree to follow all treatment recommendations and understand that Essential Aesthetics & Wellness cannot guarantee specific outcomes.

Separate informed consent forms may be required for individual treatments.

Client information will be maintained in accordance with applicable privacy laws and professional standards.

Health information will only be used for treatment, payment, healthcare operations, or as otherwise permitted or required by law.
